They help us determine which scholarship types your student is eligible to receive. A "Yes" response to any question along with a corresponding verification form (completed correctly) INCREASES A STUDENT'S POTENTIAL FOR SCHOLARSHIPS.

  • This student will be accepting ESA funds for the 2023-2024 school year, but we are applying to APESF to hold any potential funding as a multi-year scholarship.
  • Did this student attend public (or charter) school in Arizona for at least one semester of the 2022-2023 and/or 2023-2024 school year?
  • Has this student received a scholarship from a School Tuition Organization other than APESF and remained in private school since receiving it?
  • Is this student a dependent of a member of the U.S. Armed Forces stationed in Arizona pursuant to military orders?
  • Does this student have an IEP, MET or 504 plan (current or expired) from an Arizona public (or charter) school?
  • Has this student ever been in the Arizona Foster Care System?
  • Was this student homeschooled in the 2022-2023 and/or 2023-2024 school year prior to enrolling in a private school?
  • Did this student attend an out-of-state OR out-of-country school in the 2022-2023 and/or 2023-2024 school year prior to enrolling in a private school?
  • Has this student previously received ESA (Empowerment Scholarship Account), but now the account has been closed?
  • Note: A yes response to any of the above questions may require verification. You will be prompted to upload documents after clicking "Submit" at the end of this form. Alternately, you may send documents to helpdesk@apesf.org.

    An email notification is also sent to applicants when APESF makes an award. APESF does not email applicants with any sort of rejection. Please add helpdesk@apesf.org to your list of approved senders to prevent correspondence from being marked as junk mail.

    Scholarships are awarded without regard to the student's race, color, sex, handicap, familial status or national origin.

    A school tuition organization cannot award, restrict or reserve scholarships solely on the basis of a donor's recommendation. A taxpayer may not claim a tax credit if the taxpayer agrees to swap donations with another taxpayer to benefit either taxpayer's own dependent.

    A variety of considerations are made, including financial need, before any scholarship is awarded. All final decisions to award scholarships are subject to the sole and absolute discretion of APESF.
